Active Duty Sponsors

Requests for medical records require a completed and digitally signed DD Form 2870 Authorization for Disclosure of Medical or Dental Information.

Requests take 30 days to process.

 

  • Step 1: View example DD Form 2870 here.
  • Step 3: Requester will receive 2 emails upon receipt of form. The first email will include Safe Access File Exchange (SAFE) website link, password, and instructions to download records. The second email will provide a password to unlock the PDF file once downloaded.
  • Step 4: Check your email inbox and junk mail daily.
  • Step 5: Once you receive the email from the SAFE site, you have 10 calendar days to successfully download records to a location of your choice.

 

 

Adult Family Members Without CAC

Adult family members without a CAC may request records by submitting a notarized DD Form 2870 electronically. The form does not need to be notarized if submitting in person. Medical records requested by those without a CAC will be placed on a CD and mailed out to the address provided or may be picked up in person at the Correspondence Office.

 

Children 18 Years & Younger

Active Duty Sponsors may request their children, 18 years and younger, medical records. The Privacy Act and HIPAA prohibit parents from picking up the medical records of children who are over the age of 18 without written permission and an identification card.

 

How to Order Kansas State Birth Certificates

The Kansas Department of Health and Environment Office of Vital Statistics has five methods available for making a request for a birth certificate: regular mail, priority mail, telephone, internet, and walk-in customer service. Each certified birth certificate costs $15. Detailed instructions are available below.

  • Telephone: Call (877) 305-8315 anytime. Payment made by credit card. Spanish interpreters available. Additional $12.50 expedited service fee. Allow 3-5 business days after receipt of request.
  • Internet: Access www.vitalcheck.com anytime. Payment made by credit card. Additional $12.50 expedited service fee. Allow 3-5 business days after receipt of request.
  • Walk-In: The Office of Vital Statistics is open 9 a.m. - 4 p.m. on weekdays.  Payment made by cash, check, money order or credit card. Allow 15-20 minutes provided there is no difficulty locating the record.
