Medical records requested by those without a CAC will be placed on a CD and mailed out to the address provided or may be picked up in person at the Correspondence Office.

Children 18 Years & Younger

Active Duty Sponsors may request their children, 18 years and younger, medical records. The Privacy Act and HIPAA prohibit parents from picking up the medical records of children who are over the age of 18 without written permission and an identification card.
