Patient Centered Medical Homes improve the care  patients receive by offering enhanced access to care and increased Primary Care Manager continuity, and by promoting patient- and family-centered evidence-based health care.


Patient Centered Medical Homes allow providers and patients to take a more proactive approach to health care, with a stronger focus on prevention. The medical home can reduce the percentage of specialty referrals and result in a PCMH team providing the needed care in one visit.

Fast Track Care

The 1st Team Nurse Clinic offers more convenient appointment options and more time with the Healthcare Team for patient education and coaching. The clinic provides basic medical diagnoses and treatments for common conditions or symptoms. Age limits apply.

Proof of Eligibility of Minors

Proof of eligibility of a minor aged 10 or older is required for medical care services at Irwin Army Community Hospital. If proof of eligibility is not provided within 30 days of service date, the patient will be billed as a non-beneficiary.


A valid ID card and enrollment in DEERS will establish beneficiary status of a minor. Parents can obtain an ID card from the Fort Riley ID Card Center, 212 Custer Ave. on Main Post, in the west wing of the basement, Room B01.


The ID Card Center operates on an appointment only system. Appointments can be scheduled online at the RAPIDS site or call (785) 239-3654 for assistance.

1st Floor, Atrium

Park in parking garage, top level for access to skywalk. Families with strollers use  garage elevator to reach "P2" and cross the skywalk.


Note: the garage elevator does not reach the hospital's 2nd floor.
