Custer Hill Health Clinic
The Medical Home Team concept is one that provides all outpatient needs under one umbrella. Each team has several providers, such as a family practice provider, physician assistant, nurse practitioner and pediatrician, and the team is assigned to particular brigades and elements. The same providers see the same Family members within the same unit to increase continuity of care and improve overall satisfaction with health care services.

Soldiers and their families in 2nd BDE and 1st STB are now receiving their care at the Custer Hill Health Clinic, located at 7672 Estes Road on Custer Hill.

Look for a new name for the clinic in the near future! As we make the move to caring for families, "Troop Medical Clinic" just doesn't quite fit anymore! We will be dedicating the clinic with a different name soon, more to come!

FAQs about Soldier and Family Care
  • Who can receive services at the Custer Hill Health Clinic (CHHC)?
  • Irwin Army Community Hospital's goal is to create medical home teams to provide Family-oriented, patient-centered services to approximately 10,000 Active Duty Warriors and their Family members.

  • What services will be offered at the Custer Hill Health Clinic?
  • Services that will be available at the CHHC include primary care, pharmacy, laboratory, radiology, physical therapy, optometry, and patient administration. Chiropractic services will also be available for Active Duty service members.

  • What convenience will be added?
  • The CHHC will provide the clinical space necessary to maintain a medically ready and protected force, while simultaneously providing a location for Family members to receive the same quality treatment in one medical home. Beneficiaries will not have to go to multiple locations for their basic health care needs.

  • How will the CHHC measure success?
  • Close collaboration between health care providers and patients will improve customer satisfaction. Success will be measured via customer comments (ICE and letter responses).

  • How will provider productivity increase?
  • The new facility provides state-of-the-art clinic space that meets the needs of modern medicine and focuses on providing high quality health care services.

  • How will the CHHC enhance cohesion?
  • The CHHC will be a facility where continuity of care will be sustained within the Family unit under one medical umbrella to increase overall quality of and access to care. Close collaboration between health care providers and patients will improve customer satisfaction and clinical successes.

  • I like my current PCM. What if I donít want to switch PCMs?
  • Patients are assigned to one PCM within a medical home team and have the option to see other providers within that team. This will lead to overall continuity and better access to care.

  • I thought that clinic was only for soldiers?
  • As of 1 July 2010, CHHC is caring for soldiers and families of 2nd BDE and 1st STB.
