Farrelly Health Clinic continues to strive to provide the best health care possible to our Soldiers and Beneficiaries. We are continuing to improve our Medical Home Team Concept and are seeking Patient Centered Medical Home recognition this year with NCQA.

If you are new to our clinic please let us know, we have a welcome packet for you at the front desk.

Your comments and concerns are always a top priority to every member of your Farrelly Team, with your input we can continue to provide praise for a job well done or focus on improvements in areas where it is needed. If you are unhappy during your visit today please notify a member of the staff, who will be happy to direct you to our Group Practice Manager, Officer in Charge, Chief Nurse or Non Commissioned Officer in Charge.

    FAQs about Soldier and Family Care
    • What are the hours at Farrelly Health Clinic (FHC)?
    • Sick Call is from 0645-0700, Primary Care, Radiology, Physical Therapy, Occupational Therapy, Opometry and Chiropractic Services (for AD) duty hours are from 0730 - 1630. Pharmacy 0730-1630, Patient Administration 1000-1400. The clinic is open Mon-Fri excluding Federal Holidays. On IACH Training Holidays, the clinic will be open from 0730-1630, no sick call.

    • Who can receive services at the Farrelly Health Clinic?
    • FHC cares for approximately 23,000 Active Duty Soldiers and their Family Members, a majority of those are assigned to 1BCT, 4BCT, DHHB, EOD and MP's.

    • What services are offered at FHC?
    • Primary Care, Pharmacy, Laboratory, Radiology, Physical Therapy, Occupational Therapy, Optometry, Patient Administration and Chiropractic Services for AD.

    • I do not like my PCM how can I switch?
    • PCM Change forms are kept at each front desk.


    • ***Important things to bring to your appointment***

      • Arrive 10 minutes early before your schedule appointment.
      • List of questions you have for your provider.
      • CAC Card or Dependent ID.
      • A list of current medications.
      • Shot records to Well Baby Appointments.
      • CYS and/or School Physical Forms completely filled out.
