Patient-centered Medical Homes A & B
Our patient-centered medical home team concept is one that provides all outpatient needs under one umbrella. The same providers see the same Family members within the same unit to increase continuity of care and improve overall satisfaction with health care services.

The changes have improved access to care, because patients can see anyone on their medical home team. The team is assigned to particular brigades and elements. Our teams are comprised of providers, such as internal medicine, family practice, physician. assistant, nurse practitioner, pediatrician and behavioral health marriage family therapist; Nursing staff comprised of registered nurse (RN), triage advise line nurse, military medic, licensed practical nurse (LPNs), certified nursing assistant (CNA). Patients can enjoy a better experience by seeing the same medical home team on a continual basis building rapport and trust.

Our teams meet regularly to keep everyone appraised of situational factors in the community that affect the health and wellbeing of personnel assigned to MEDDAC, DENTAC, VET Activity and their dependents as well as retirees and their family members. Our teams can deliver better health care by having the total health picture of the Family, Soldier, and unit.

Patient-centered Medical Home A & B have ancillary support services that include pharmacy, laboratory, radiology, physical therapy, occupational therapy, optometry, Chiropractic services, behavioral health, nurse case management, and patient administration.

    Types and Ages of Patients Served
    The Primary Care Clinics provide acute care, chronic care, and health maintenance to patients of all ages. The clinic serves active duty, and their dependents, as well as retirees and their dependents, while they are military beneficiaries and enrolled in Tricare. Patients who are not enrolled in Tricare Prime and those beneficiaries over 65 are served on a space available basis.

    Walk-ins
    The following services will be offered to all TriCare Prime beneficiaries from 0800 to 1530:
    • Blood pressure check (will need to sit in clinic for tem minutes prior to check)
    • Weight check
    • B12, Depo-Provera injections (if previously ordered by an IACH provider)

    Patients under age 18
    Patients age 17 and younger must be accompanied by a parent or legal guardian in accordance with FR MEDDAC 40-16, Consent by Patients for Medical Care. Exceptions are as follows: they are an emancipated adult or they are requesting treatment for a sexually transmitted disease, drug abuse or addiction, mental health, or treatment or procedures for emergency care for 16 years of age or older where no parent or guardian is immediately available. The child can be accompanied by anyone over the age of 18 with a notarized medical power of attorney signed by the parent or guardian. This can be obtained from the Customer Service Advocate (CSA) or JAG.

    Mission
    The Medical Home Clinics provide care to all ages of patients from newborn to geriatric retirees. Services include initial and ongoing evaluation, diagnosis, treatment, patient education, follow-up and immunizations.
